Replace dynamic distance calculation for table plugin with pre-calculated distances on shortcuts, avoiding unpacking cost. 
Adds approx 10% to total data size.  Speeds up large table requests by 2 orders of magnitude.

* optionally include condition and via node coords in InputRestrictionContainer

37794a5e8a Change traffic CSV field value from weight to rate 
and make the value required.

If the weight name is 'duration' than the rate value
can be computed as speed / 3.6
